Home Nursing

We assess each patient's condition individually to determine the most appropriate care required. Careplan goals are discussed with patients/next of kin and progress are monitored. For patients requiring support from the community or acute care institution, nurses also coordinate and integrate care to make it seamless for the patients.

We provide nursing care in the following areas:

  • Health assessment including monitoring of blood sugar and blood pressure
  • Medication management
  • Administration of injections/loading of insulin
  • Nasogastric tube management
  • Urinary catheters management
  • Wound and stoma care
  • Removal of stitches/staples
  • Negative Pressure Dressing
  • Patient/caregiver training
  • Manage home bound patients with Bullous Pemphigoid (In collaboration with National Skin Centre)
  • Manage patients with exit site infection of Peritoneal Dialysis (PD) Catheter (In collaboration with National Kidney Foundation Singapore (NKF))
