Advance Care Planning

Advance Care Planning (ACP) is a way to ensure that your future health and personal care  preferences are known and respected, especially if you are unable to communicate during a medical emergency. ACP gives you peace of mind, knowing your choices will be honoured when it matters the most.

It involves:

  • Reflecting on your values, beliefs, and preferences regarding care
  • Appointing a trusted person (Nominated Healthcare Spokesperson) to make decisions on your behalf in a health crisis
  • Documenting your preferences to guide future healthcare decisions
  • Your ACP is easily accessible to healthcare providers via Singapore’s National Database
  • ACP is not legally binding and can be updated if your preferences change over time

 

Who should do ACP?

Anyone aged 21 and above, regardless of health condition and has mental capacity

 

Why should I do ACP?

  • To ensure your care and treatment preferences are respected during a medical emergency
  • To reduce the stress for your loved ones, knowing they won’t have to make tough decisions in a crisis
  • To complement important legal documents such as the Lasting Power of Attorney (LPA) and Will, providing a complete plan for your future

 

How to do an ACP?

  • Schedule an appointment with our trained ACP facilitator (in-person or via video call)
  • Expect at least one hour for the session, and more if you need additional time to reflect on your preferences
  • Book your session easily through My Legacy booking site.

 

What do I need to prepare for ACP?

Take some time to reflect on these important questions:

  • What does “Living Well” mean to me?
  • What are my biggest health concerns or fears for my future?
  • Who do I want to appoint as my nominated healthcare spokesperson?
  • How and where would I want to be cared if I need help with daily activities?
  • If faced with a health crisis, what type of care would I prefer? Comfort care vs life sustaining treatment
