Job Responsibilities:

• Responsible for case work management involving systemic assessment and intervention for the well-being of patients and family members.
• Perform referrals to other service providers and conduct home visits when necessary.
• Work closely with multi-disciplinary team members.
• Support projects and programmes.
• Attend training and case work supervision.

Job Requirements:

• Degree in Social Work or a Graduate Diploma in Social Work.
• Registered with Singapore Association of Social Work.
• Experience in means testing and healthcare setting is advantageous but fresh graduate may apply.
• Conversant in local dialects and/or Malay, Mandarin, Tamil.
